Job Description :
LifeStream is a behavioral health and social services organization that provides high-quality treatment, education, care management, rehabilitation, child welfare, primary care and homeless services to children, adolescents, and adults. Located in Central Florida, LifeStream primarily serves the residents of Lake, Citrus and Sumter Counties, with additional programs located in Orange, Marion, Hernando and Hillsborough Counties.

High School Diploma or General Education Degree (GED). Associates Degree preferred.
Six (6) months to one (1) year experience in the human services field required. Preferred experience working with community resources, local providers, and facilitating/documenting referrals.
Satisfactory Driving Record while possessing a Valid FL Driver’s License
Level II Background Clearance
JOB SUMMARY: This is a support position with primary responsibility to provide assistance to case managers in the performance of specified job duties.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
Complete referrals as needed for the child(ren), caregiver(s), and parent(s) within 5 business days of the need being identified.
Complete and submit Diagnostic and Evaluation Requests as well as all required documentation to the Utilization Management Department within 5 business days of the request.
Make and document in Florida Safe Families Network (FSFN) monthly contact with all service providers; a detailed summary to be sent to assigned care manager and their supervisor within 3 business days of the contact.
Complete Interstate Compact for Placement of Children/Out-of-County Services requests with all required documents within 2 weeks of request. Submit the packet to the Point of Contact for processing once reviewed and approved by the assigned case manager and their supervisor.
Maintain and document in FSFN monthly contact with the Out of County Services worker. A detailed summary will be provided to assigned case worker for review.
Obtain Medical/Dental records for the file as needed. Scan the reports into FSFN once reviewed and initialed by the assigned case manager. Complete referrals and schedule appointments follow-up as mandated in the medical/dental reports.
Review Court Orders (Docket sheet) and create a tracking mechanism to ensure all tasks are completed within 2 weeks of receipt of the order.
Compile all required documents for SIPP/STGH admission within 2 weeks of the request.
Compile subsidy packets for adoption case managers and supervisor review within 2 weeks of request.
Other duties as assigned.

ESSENTIAL SKILLS: Abide by core values and mission statement of agency. Excellent computer skills (Microsoft Office Suite). Knowledge of basic office equipment. Excellent interpersonal and communications kills (verbal and written). General office equipment such as phone, scanner, copier, computer, etc. Ability to occasionally work a flexible schedule, including evenings and weekends. Experience identifying and communicating with resources and community partners. Experience completing referrals and tracking until implementation.
